Protocol Overview

Responsible CORE protocol planning includes having the Antidote Pen on hand.

The Antidote Pen is a Priapism Response Support pen providing 10 doses of response compound — 20 clicks per dose — for use in the event of a prolonged or painful erection that does not resolve within the expected timeframe. Priapism — an erection lasting more than 4 hours — is a medical emergency. The Antidote Pen is not a treatment substitute for emergency medical care; it is an intermediate support tool for responsible CORE protocol planning. All CORE users should have an Antidote Pen accessible before beginning any CORE protocol.

Step 02

How It Works

The Antidote Pen contains a compound that acts through the opposing vascular pathway to the CORE Trimix mechanism — producing local vasoconstriction to reduce arterial inflow and support resolution of a prolonged erection. This reversal mechanism is well-established in clinical management of priapism and is the pharmacological basis for responsible CORE protocol safety planning.

Administration

Subcutaneous injection at the confirmed administration site if a prolonged erection occurs and does not resolve. Administer 20 clicks (one dose). If no resolution occurs within 30 minutes, seek emergency medical care immediately. The Antidote Pen is a response support tool — it does not replace urgent medical evaluation for priapism lasting more than 4 hours.

Titration Note

The Antidote Pen is not titrated. It is used at the confirmed single-dose level (20 clicks) in a response situation. If you have any questions about appropriate use, contact your private access support before your first CORE protocol session.

Who This Is For

Every CORE protocol client. The Antidote Pen should be ordered alongside any CORE Trimix pen. It is not optional for responsible CORE protocol use. Priapism risk exists across all CORE compounds at any dose — having response support immediately accessible is part of correct protocol setup.

What Is Trimix?

Trimix is a compounded three-compound intracavernosal formula that has been used in clinical urology for over three decades. Where oral medications like sildenafil (Viagra) or tadalafil (Cialis) work systemically through the bloodstream — producing effects that depend on sufficient nitric oxide production and intact vascular pathways — Trimix acts locally and directly at the target site. The three compounds work through complementary, synergistic mechanisms to produce a reliable, titratable local erectile response regardless of the systemic factors that limit oral medication effectiveness.

What Each Component Does

Primary vasodilator

Alprostadil (PGE1)

Prostaglandin E1 binds directly to receptors in cavernosal smooth muscle, activating adenylyl cyclase and increasing cyclic AMP — which relaxes smooth muscle, increases arterial inflow, and initiates the erectile response. Alprostadil is the active agent in multiple FDA-approved topical and injectable formulations for erectile response support. As the primary component of Trimix, it produces the foundational local vasodilatory effect.

Non-selective PDE inhibitor

Papaverine

Papaverine inhibits phosphodiesterase enzymes non-selectively — preventing the breakdown of both cyclic AMP and cyclic GMP in smooth muscle cells. This amplifies and prolongs the relaxation signal initiated by alprostadil, extending the duration and reliability of smooth muscle relaxation in the corpus cavernosum. Papaverine has been used in intracavernosal therapy since the 1980s and is the oldest clinical compound in the Trimix combination.

Alpha-adrenergic blocker

Phentolamine

Phentolamine blocks alpha-adrenergic receptors — the receptors responsible for sympathetically mediated vasoconstriction. Under stress, anxiety, or sympathetic nervous system activation, alpha-adrenergic tone can counteract the vasodilatory effect of the other compounds. Phentolamine prevents this, ensuring consistent smooth muscle relaxation regardless of psychological or sympathetic state. This is why Trimix can produce reliable response even in contexts where psychological factors have previously limited effectiveness.

Why Titration Matters

CORE Trimix protocols require careful individual dose titration because the response to intracavernosal compounds varies significantly between individuals — and the consequence of over-dosing is priapism, a medical emergency. There is no universal starting dose. Individual response depends on baseline vascular tone, compound sensitivity, concurrent medications, and psychological factors. Private onboarding confirms your starting dose before your first session. Titration increases this dose incrementally across sessions until your effective dose is identified. Do not skip titration or assume a dose from community forums — the risk is not the ineffectiveness of an underdose, but the danger of an overdose.

The Antidote Pen Is Not Optional

Every CORE protocol client should have an Antidote Pen accessible before beginning any session. This is not a precaution for high-dose users only — priapism risk exists at any dose and in any session, including those at established doses that have previously produced normal responses. The Antidote Pen provides immediate pharmacological response support while emergency medical care is being arranged.

Understanding Priapism Risk

Priapism — a prolonged, painful erection lasting more than 4 hours — is a medical emergency that can result in permanent vascular damage if untreated. All CORE compounds carry this risk at any dose level. The risk is managed, not eliminated, through careful titration and having the Antidote Pen immediately accessible. Seek emergency medical care immediately if an erection becomes painful, lasts longer than 4 hours, or does not respond to Antidote Pen administration within 30 minutes.

Before You Begin

  • Have your Antidote Pen accessible at all times
  • Always alternate injection sides — 9–11 o'clock (left) or 1–3 o'clock (right)
  • Never inject at 12 o'clock or 6 o'clock positions
  • Start at the lowest recommended dose
  • Seek urgent medical care immediately if erection lasts more than 4 hours
